1. ROLE
This role offers a fantastic opportunity to be a member of a
multi-disciplined team using holistic approaches to support Depaul
service users. This post holder will be responsible for liaising with
the staff team and working with service users to provide specialist
support in relation to mental health issues. The post holder will
provide inreach Dual Diagnosis (Mental Health and Addiction) support
to service users in homeless and shielding accommodation.
2. KEY AREAS OF RESPONSIBILITY
* To establish and maintain a dual diagnosis role within the Health
support team.
* To engage and work proactively with services users who have dual
diagnosis issues, offering specialist support to around their mental
health and addiction needs.
* To work with service users placed in various homeless Private
Emergency Accommodation and Shielding units (i.e PEAs) Health support
team assessing their needs with particular focus on mental health,
addiction and resettlement needs.
* To plan and facilitate appropriate strategies and interventions to
respond and meet those needs in a structured way.
* To case manage and support the service users and accommodation
providers with care planning and the development of support plans.
* To collaborate, develop and document appropriate care in
conjunction with Health Support Team members and partner agencies. To
support the ongoing care and case management provided both partner
agencies and accommodation providers around mental health and
addiction needs.
* To advocate for appropriate move on options for service users with
mental health issues and to facilitate and carry out resettlement
assessments with clients.
* To support appropriate assessment, transfer and hospital discharge
info shielding units and implement appropriate care plans to support
resettlement.
* To promote, support and monitor crisis alcohol and benzo detox,
drug stabilisation, and provide appropriate referral for residential
treatment options in conjunction with clinical staff, GPS and harm
reduction workers.
* To support clinical staff and accommodation providers with issues
relating to the implementation of symptom monitoring, testing and
isolation pathways to reduce transmission of Covid **Apply on the

* To support Covid-**Apply on the website** contact tracing, testing
and sweeping of residential facilitates as directed by public health
and HSE Covid Response team protocols and procedures
* To accompany service users to access appointments and engage with
treatment options
* To develop and maintain the ongoing liaison between the
accommodation provider and mental health and addiction services (i.e.
? Addiction treatment clinics, Community GPs, community mental
health teams, homeless specific mental health teams, other
professionals and Health services).
* To advocate for psychological / psychiatric assessments where
appropriate and in the interest of the service user in conjunction
with nursing colleagues and inreach teams.
* To promote good mental health and wellbeing for services users.
* Maintain a high standard of care and continuity with individuals
in relation to their health and wellbeing and addiction needs refer to
appropriate services as necessary.
* To ensure all quality assurance policies and procedures are
adhered to.
* To maintain open communication with staff using localized
communications protocols (Handovers, clinical documentation and IT
systems)
* To observe Health & Safety Procedures in particular regular review
of activity risk assessments and maintain a safe working environment
for staff and service users.
* To observe clear professional boundaries in relations with the
service users we work with.
* To undertake and maintain all core training as provided by the
organisation commensurate with the role.
* To respect service users right to confidentiality in our
relationship with them as per the Organizational Confidentiality
Policy and Code of Practice.
* To undertake your role in a professional manner maintaining a high
quality standard of work, and work in accordance with the aims,
values, and ethos of the organization.
* To undertake any additional duties that may be required which are
commensurate of the role in consultation with your line manager.
ADMINISTRATIVE RESPONSIBILITIES:
* To keep and maintain accurate and up to date records in relation to
care planning and other interventions
* To attend and participate in weekly team meetings, training,
webinars/conference calls, supervision and other activities as
directed by the organization or HSE Homeless Covid Lead.
* To participate in data collection, reporting, and research
initiatives as directed

3. PERSON REQUIREMENTS
* At least one year of working, as a case manager with service users
with complex needs and co-ordinating support plans for service users.
(Essential)
* At least one year of regular use of IT systems with a good
proficiency in Excel & Word (essential)
* Experience of working in liaison with other agencies within the
voluntary and statutory sectors (essential)
* A minimum of 2 years? experience working in the homeless sector
or another related field, with people requiring mental health support
(desirable)
* A qualification in nursing, social care, social work,
psychology/psychotherapy, addiction studies or other related area
would be desirable.
* An understanding of why people become homeless and the issues they
experience, particularly in relation to people requiring mental health
and addiction support
* Have an understanding of harm reduction and low threshold working
and working within these values as part of a multidisciplinary and
multiagency team
* The ability to communicate effectively both verbally and in
writing
* Ability to work independently, off-site/inreach working as well as
the and as part of a team
* An understanding of relevant models of interventions that can be
applied in working with people experiencing homelessness
* Experience responding to crisis, knowledge of appropriate actions
and use of initiative to support others
* An ability to liaise effectively with other service providers and
represent the organisation externally
* An understanding of the statutory and voluntary sector resources
available to homeless people
* An understanding of the importance of an organisations policies
and procedures and experience working with these guidelines
* Good computer skills including experience using client management
computer software
* A commitment to and an understanding of equal opportunities and
